Sinus maxillaris surgery

Sinus maxillaris (Maxillary sinus) are anatomical cavities located above the upper jaw and in close proximity to the molars. In oral surgery practice, there are several instances where intervention in these cavities becomes necessary due to various reasons. In some cases, teeth are partially or completely situated within the maxillary sinus, creating a connection between the bottom of the sinus and the oral cavity during tooth extraction (oroantral communication). In such cases, this established connection must be surgically closed using different methods. Additionally, when the inflammatory process from infected tooth roots extends into the maxillary sinus, causing odontogenic sinusitis, and an oroantral communication is present due to the extraction of the causative tooth, the infected maxillary sinus must be surgically treated and closed. In such situations, the method applied should not involve radical surgeries like “Caldwell Luc” under any circumstances. Furthermore, during the extraction of upper wisdom teeth, the displacement of the tooth into the maxillary sinus can also necessitate intervention in the maxillary sinus.

In addition to the aforementioned scenarios, insufficient mucous secretion is often encountered in the posterior region of the upper jaw after tooth loss. During the restoration of these missing teeth with dental implants, certain difficulties may arise due to the insufficient mucous quantity.

To address these difficulties, sinus floor elevation procedure, also known as sinus lifting surgery, can be performed. Through the application of this method, mucous membrane elevation is conducted to create space corresponding to the size of the planned implant, allowing successful implant placement. The surgery can be performed using either open or closed techniques, depending on the clinical situation. Both types of procedures can be carried out under local anesthesia. Depending on the clinical situation, sometimes this surgery is performed in the same session as the placement of implants, while other times it is conducted as two separate procedures.
